Chronic Care Management
Chronic Care Management is a scholarly work, published in 2014 in ''Value in Health''. The main subjects of the publication include Czech, health care, medical guideline, and medicine. The aim of this study was to suggest a management care system of the chronically ill in the Czech Republic, which should increase the quality of medical care and decrease the costs of it.
